Marking the body chart
The chart shows a real anatomical figure, front and back. Drop a marker where the finding is, and set:
- Type — pain, tenderness, numbness, or referred symptoms.
- Severity — mild, moderate or severe.
Add as many markers as you need. They're saved with the note, so the picture sits alongside your written findings and travels with the record.
Pain score
Record a 0–10 pain score (NRS) on the note. Tracked over a course of treatment, it's a simple, recognised way to show change — and it pairs naturally with formal outcome measures when you want something more structured.
