Write a note
Open the appointment and start the note. The fields you see depend on your note format — a physiotherapist gets SOAP, a psychologist gets DAP, a counsellor gets BIRP, a dietitian gets ADIME. Atlacare picks the right one from your profession, so you don't build a template before your first patient. See which note format your profession uses.
Your work autosaves every 30 seconds, and a draft badge shows while a note is unsaved, so an interruption between patients won't lose anything.
Body chart and pain score
For professions that use them, you can drop markers on an anatomical body chart — pain, tenderness, numbness or referred symptoms, each with a severity — and record a 0–10 pain score. These sit alongside the written note on the record.
Locking and versioning
When you sign a note it locks. If you need to change a locked note, Atlacare keeps the original and records the amendment as a new version rather than overwriting it — so the record stands up if you're ever asked for it.
Who can see it
Every note has a visibility setting that controls whether the patient sees it and which staff can open it. This is important enough to have its own guide — see Note visibility: patient-visible, internal and confidential.
Notes marked patient-visible appear in the patient's portal; everything else stays inside the clinic.
