Forms let patients fill in their details, health questions and consent before they arrive, so you're not doing it on paper in the first ten minutes of the session. You'll find them under Forms in the sidebar.
Forms are included on the Solo, Clinic and Practice plans. On the Free plan the Forms page shows what's available, but you can't build or send forms there.
Start from the library
Open Forms and choose the Library tab. It holds 16 ready-made forms, including a new patient intake, GDPR consent, MSK assessment, consent for under-16s, video consultation consent, PSFS, NDI, PHQ-9 and GAD-7. Filter by discipline, then click Add to my forms on any you want. The copy is yours to edit.
Build your own
- On Forms, click New form.
- Give it a name and, if you like, a description.
- Click Add field and choose a field type: short or long text, number, email, phone, date, yes/no, single or multiple choice, dropdown, scale, checkbox, signature, section heading or statement.
- Turn on Consent form if the patient must sign it (for GDPR or treatment consent).
- The preview on the right shows exactly what the patient will see. When you're happy, click Publish form.
Your work saves as a draft while you build. A draft can't be sent until it's published: on the Forms list it shows Draft - publish first instead of Send.
Send a form to a patient
- On Forms, find the form and click Send.
- Search for the patient by name, email or phone.
- Add a Message to patient (optional).
- Click Send form.
If the patient has an email address, they're emailed a secure link straight away. You also get the link to share another way (for example by text or WhatsApp). The link is personal to that patient, needs no password, and expires after 30 days.
Send a form automatically when a new patient books
- Go to Settings → Patient forms.
- Turn on Auto-send a form when a new patient books.
- Under Form to send, choose a published form.
- Optionally tick the reminder, so the patient is reminded about 24 hours before the appointment if they haven't completed it.
- Click Save.
This applies to a new patient's first booking, whether you book it or they book online. A patient who has already been sent that form isn't sent it again.
Where the answers appear
- On the patient's profile, the Forms card lists every form sent, with Awaiting completion, Completed or Cancelled. Click a completed form to read the answers. For a form still waiting, you can Resend it or Cancel the request.
- Patients with the patient portal also see forms waiting for them under Forms in the portal.
Who can do what
- Owners and managers can build, edit, remove and send forms, and set the auto-send form.
- Practitioners can send forms but not build or edit them.
- Receptionists can view and preview forms.
To let brand-new patients enter their own details before you've created a record for them, use the new-patient registration link.
